Admin and Cust. Serv. Coord.

Our Administrative and Customer Service Coordinator is central to our innovation integrated Health and Wellness Clinics.  
 
Vivacitas is a new idea operating healthcare clinics in independent and assisted living communities. Vivacitas provides senior adults living in these communities with easy access to clinic services, primary care physicians, nurses and labs. 
The Admin and Customer Service Coordinator is a hands-on position, responsible for the customer service and administrative functions of multiple clinic locations. 

Position Responsibilities:

Acting as the customer service representative for patients and patients’ family and representatives.
Supporting new patient on boarding and helping with education events and clinics 
Managing pre-and post-appointment patient follow up, support in documentation, 
Holding responsibility for patient experience at the clinic
Working proactivity with clinicians and community staff to help resolve issues/conflicts/problems of patients.
Scheduling patient appointments, providing reminders and follow up
Managing physicians’ calendar 
Keeping inventory and supply management
Helping with third party follow up including pharmacy, clinical specialties and DME providers
Opening and closing protocols 
Preparing activity reports 
Ensuring that all privacy and HIPAA rules and regulations are in compliance and followed in the clinics
Participating in staff meeting and trainings 
  
Education and Skills:
 
Experience working with older adults
Strong organizational and communication skills
Strong Customer Service skills and experience 
Ability to be onsite and drive to 5 clinics
Ability to work individually and as a team
Bachelor’s degree in business, healthcare or related degree, desired.

IP Strategist

Vivacitas is seeking an experienced leader to serve as director of Intellectual Property (IP), Licensing. You will have responsibility for the early identification of IP that is eligible for licensing, then creating licensing revenue via initiation, negotiation and consummation of IP licensing opportunities.


Responsibilities

IP distillation and patent licensing activities in analytics and healthcare technology.
Negotiate and consummate license agreements, delivering predictable and steady licensing revenue and brand value.
Expertise with the venture capital sector at the GP level.
Skills in IP valuation in analytics and healthcare technology.
Advise the leadership team on IP matters in general, including the Board

Experience Requirements

Position Summary:


The Physician will act as a primary care physician for all patients participating in the Vivacitas Medical Group located in five (5) Assisted Living and Independent Living Facilities in the Santa Rosa area.
Under the direction of the Chief Medical Officer support the development of comprehensive care plans, intervention protocols, preventative care and quality improvement systems. 


Primary Duties and Responsibilities:

Act as primary care physician and coordinating all aspects of clinical /medical care 
Obtain medical, social and family histories with the adult children
Perform physical exams and cognitive screens on all patients.
Routine labs to be drawn as well as emergent ones to help with diagnosis
Provide health education and counseling.
Participate in ongoing wellness clinics (i.e. flu shots clinic, immunizations, vitals)
Create effective acute, chronic, and complex care plans
Coordinate care with specialists for high risk patients. 
Create care coordination mechanism among the patient, family and community staff to improve patient experience, clinical outcomes and regulatory compliance 
Provide accurate and timely documentation for billing to Medicare, Managed Care, and other insurance providers
Understand and model high customer service.
Participation in Quality Improvement program.
Provide case management with other health care team members.
Drive growth via patient education seminars and direct engagement with patient and family members

Vivacitas is operates healthcare clinics in independent and assisted living communities in and around Santa Rosa, Vivacitas provides older adults living in these communities with easy access to clinic services, primary care physicians and nurses, and basic labs. 
 
Position responsibilities include:
 
Provide direct patient assessment and care
Participate in preparing and implementing care coordination plans 
Conduct wellness/preventative checks including vitals, routine labs and standardized assessments 
Coordinate with third party health care providers and community staff
Respond timely to changes in condition 
Communicate with patients, families and community staff to coordinate care
Participate in interdisciplinary care planning and education
Provide educational sessions to staff and patients
Support potential patients in gathering information and completing any required paperwork
Ability to operate under supervision of Physician staff
 
Education and Skills: 

Strong clinical skills working with geriatric clients,
Experience with direct patient care 
Experience with lab and blood draws, a plus
Geriatric experience – including physical and cognitive assessments and interventions experience with medication evaluations
Strong documentation and clinical assessment skills
Strong organizational and communication skills
Knowledge of clinical best practices and procedures, as well as laws, regulations,
Experience in resolving conflict, creating a positive clinical experience, and dealing with patients, families and others with respect and tact.
Ability to be onsite and drive to 5 clinics
Ability to work individually and as a team 
Current and Active Licensed nurse practitioner with Bachelor’s degree in nursing or related degree preferred
